QUESCREN Education and Vitality Forum 2023

Waiver

Photographs, videos, and voice recordings will be taken at the forum and therefore by submitting this permission form and checking the box below, I also hereby: Give permission to QUESCREN at Concordia University to use my image, both in photographic as well as in electronic form for the purposes of creating educational content and promoting QUESCREN activities. I understand that my image and voice may be used in both written and electronic communications and presentations. I waive any and all copyright, including moral rights that I may have in my image and voice only with respect to their use for the purposes of creating educational content and promoting QUESCREN. I further warrant that I own all the copyright and moral rights allowing me to execute the present waiver.

If I wish to revoke this photo/video/voice release, I can do so by informing the organizers upon check-in at the forum (at the registration desk).

<h2>Renonciation</h2><p>Des photographies seront prises à l’occasion du forum. De plus, certaines séances feront l’objet d’un enregistrement audio ou vidéo. Par conséquent, en m’inscrivant à l’événement et en cochant la case ci-dessous, j’accorde à QUESCREN et à l’Université Concordia le droit d’utiliser mon image, qu’elle soit sous forme photographique ou électronique, dans le but de promouvoir les activités du réseau. Il est entendu que mon image et ma voix peuvent être utilisées dans des communications et présentations écrites et électroniques. Je renonce à tous mes droits d’auteur, y compris aux droits moraux que je pourrais avoir sur mon image et ma voix, mais uniquement en ce qui concerne leur utilisation pour créer du contenu éducatif et promouvoir QUESCREN. Je certifie en outre posséder tous les droits d’auteur et les droits moraux m’autorisant à exécuter la présente renonciation.</p><p>Si je souhaite révoquer cette autorisation de photo/vidéo/voix, je peux le faire en informant les organisateurs lors de l'enregistrement au forum (au bureau d'inscription).</p>
